You can use Smiley Digital Embed to collect feedback in a Sharepoint environment. 

1. Go to Admin > Surveys.
2. Go to the survey and click its name.  
3. Scroll to the correct experience point and select Get script and assets.
4. Select Smiley Digital Embed and then Web.
5. Copy the Smiley Digital code. 
6. In SharePoint, click Edit to Edit the page
7. Add a new Web part to the page
8. Use the Embed feature on Sharepoint to embed the code.
